National Changing DiabetesSM Program Introduces DiabetesXChange.org

On the heels of National Diabetes Month, the National Changing DiabetesSM Program unveiled DiabetesXChange.org, a promising new Web site that will serve as a central online national clearinghouse for groundbreaking initiatives in diabetes care, prevention and management in the United States. The Web site is the premier site for the diabetes community to share information and learn about the growing number of successful diabetes programs–from small community-based initiatives to large government projects and corporate wellness programs.

“There are a significant number of innovative diabetes initiatives underway across the nation that are showing positive results; what’s missing is a central location where this information can be accessed and shared,” said Dana Haza, Senior Director of the National Changing Diabetes Program at Novo Nordisk. “We created DiabetesXChange.org to fill this gap. We want people who care about changing diabetes to visit DiabetesXChange.org to read about innovative interventions and lessons learned, and to contribute more initiatives to the site.”

The site, which is sponsored by Novo Nordisk, features a diverse collection of promising initiatives submitted by the diabetes community, health care providers, employers and government agencies. Programs and initiatives focus on a variety of interventions such as telemedicine, coaching, health information technology and certified diabetes educator engagement. DiabetesXChange.org is not a commercial site and, as such, will not endorse any specific diabetes therapy.

“We have more than 20 million people in the U.S. living with diabetes and this number is on the rise,” said Donna Rice, president of the American Association of Diabetes Educators (AADE). “I commend the National Changing Diabetes Program for creating an online resource to showcase programs and interventions working to improve the lives of people with diabetes.”

One company sharing its initiative on DiabetesXChange.org is Gulfstream Aerospace Corporation, which employs more than 7,900 people. The company launched an initiative at its Savannah location called, Partners In Quality that, in addition to other disease groups, is designed to ensure that all of its employees with diabetes and covered dependants receive the proper treatment from their primary care physician and avoid the more expensive hospital costs of treating co-morbidities associated with poor diabetic control.

At the community level, the city of Asheville, NC, a self-insured employer, created The Asheville Project. Since 1996, the city, in partnership with Mission Hospitals and the North Carolina Association of Pharmacists, has been providing education and personal oversight for city employees with chronic health problems including diabetes. The program provides employees with diabetes with intensive education about the disease and matches them with community pharmacists who monitor their condition and make sure they are taking medications correctly. Favorable outcomes reported include a reduction of annual sick leave by employees with diabetes, a decrease in annual medical claims and a significant number of employees achieving and maintaining their cholesterol and A1c goals.

“It is important to have a resource where pharmacists can go to learn what other pharmacists are doing,” said Kelly Ragucci, Associate Professor, College of Pharmacy, Medical University of South Carolina. “These patient-focused programs ensure true continuity of care for anyone working in the field of diabetes education and management.”

DiabetesXChange.org offers a searchable database for users to pin-point a specific diabetes program or browse a variety of initiatives sorted by type and community served. Contact information is provided with the initiatives to encourage further information sharing. The site also includes a submission feature for ongoing contributions to the site along with a resource center with links to other diabetes online resources, research reports and National Changing Diabetes Program partners.
